Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

[review] => We had an outstanding experience from the moment my mom arrived to even after mom left. We understand that staff members are very, very busy. But you could not tell, becasue everyone had smiles all day, spoke pleasantly to each other, and made sure my mom never waited long the few times she rang her call bell. The medical care team were very attentive and answered every question we had. PT/OT got my mother's strength up so I felt safe having her come home. The admin side of the house was just as supportive and coped well with different family members calling. Once my mother got stronger, she wanted a very quick discharge date. The entire staff worked hard to get all the orders and services arranged for my mother to continue with home care. They really put the patients needs and wants ahead of what is convenient for them. Should we ever need rehab or nursing home services in the future, Arbor Trail will be our first choice.

[review] => My mom was a long term resident at Arbor Trail from August 2015 until June 2023. I picked Arbor Trail after spending two days interviewing almost a dozen other facilities throughout Citrus County. She loved the staff, who took exceptionally good care of her. The facility was always clean; I noticed that rarely if ever would I smell urine or feces when walking the halls of the facility (unlike facilities I had visited in Michigan when my grandmother was in care). Arbor Trail also takes pride in their low turnover rates among the staff, which really makes a difference to the longer-term residents. Many of the same folks who worked at Arbor Trail when Mom moved in were still with the facility after her passing. They were exceptionally helpful in every aspect of her care, including the diligent management of her finances. The staff and the business office were always very trustworthy, and helpful to conserve as much of mom's money as possible.

Caring personnel and great therapy made all the difference in the world. Dawn and Lana were beneficial in finally getting my granddaughter’s transportation to the Mayo Clinic which was denied by the insurance company. [review] => My mom had a stroke and she couldn't see anymore or walk, I chose this rehab for all the good reviews. Well her first couple of days was a night mare her problem was the people she shared a room with who had their TV blaring all day and night. I learned no one can tell the other room mate to turn down the volume on the TV. Nurses can't do anything, CNA couldn't do anything either, I was in tears. Had a meeting with directors and social services it also included my mom and me in the meeting. Really thought things worked out
moved my mom to another room but that roommate was the night mare from hell, especially that night. Had to go back to the facility at 1:30 in the morning because my mom called very up set wanting me to come get her. A stroke patient needs peace and quite and be able to sleep.
When I arrived I met with the shift manager and she had already moved my mom in another room so she could get some sleep, good thinking and Mrs. Benjamin was very caring and understanding. I checked in on mom and she was resting. I was fine with the results for that night, except for the nurse she needs to learn every patient deserves respect and treated with dignity whether she was a resident or not. Thank you again Mrs. Benjamin again for all your help and understanding.

I have been out of commission for years and other facilities kept me around just to fill a bed, the administration at one actually told my family i would never improve, stuck me in a bed and got me a geri-chair only at my familys insistance that i have the option to get up. Arbor Trail's OT and PT department has been working with me since i got here, and almost immediately got me a wheelchair! Not only can i get out of bed, but i can zoom around the facility on my own!!! They are now starting to work with me to use a slideboard for transfers!!!!!

[review] => Making the decision to place a loved one in a SNF is not an easy task but one that becomes necessary. When my dads health began to decline it was obvious he needed extra care around the clock and it was not something we could provide for him. He was placed in one facility and as soon as possible we removed him and placed him in Arbor Trail. Before doing so I did my research on local SNF and decided to tour Arbor Trail. When we got there I felt such peace in placing my dad there! The admission team, social services, business office, nursing staff, the physicians, and administrator were all so very welcoming and explained in detail what to expect with my dads care, and they explained the process of transporting him from the facility he was in to Arbor Trail.
I can't thank the team enough for all they have done for my dad from the day he became a resident. I can call any time and get updates on him, I can visit whenever I want to, and answer any questions I have. It takes a special team to ensure each need the residents have are not only met but met with dignity, empathy, and compassion. So for me, they go above and beyond and my dad is thriving when I was so fearful. A year later, he's still with them and it's because of the team that surrounds him.

When he was admitted to Arbor Trail he was unable to feed himself, he was very thin and weak, and his overall outlook was not so good. Now, after working with therapists he's able to feed himself, he's gained weight, and his is more aware of his surroundings! We are thankful for all who work there!

[review] => One of the hardest decisions our family had to make was placing our mother into a nursing facility, fortunately the staff at Arbor Trails made this experience so much easier. They are so kind and understanding from the nurses to the business office everyone at Arbor Trails seems to truly care for your loved one. The care she received there was remarkable. When she was able to be discharged, their Social Services dept went above and beyond. They provided information about everything a family in our situation was experiencing and even things we weren’t aware of. I highly recommend Arbor Trails if you’re needing nursing home assistance.

[review] => 1 Star is worth more than I could give. Do NOT send a loved one to this facility. My mother was transferred there for therapy with an asymptomatic Covid diagnosis. She received zero therapy. She was stuck in a room with little interaction. Was never seen by a physician. she was complaining of a “bunch” on her hip which turned out to be a LARGE mass. They never return phone calls. Her walker and other items that were transferred there with her are missing. And there social services department doesn’t return calls. This place is the waiting room to heaven. Ohhhhh I almost forgot the worse one yet. “Nurse” Bob changed an open wound dressing with no gloves. Thanks Bob mom now an infection!!!!!

The days she wasn't at work I had the people that work under her to keep leaving messages on her desk with my phone number not once did I receive a call back so today is January 6 23 and I would like to state I called arbor trails one last time as for hopes to speak with the dietary aid women but the front desk lady asked if I worked there and I said no I'm just trying to get my my message to her to let her know I've been trying to get a hold of her and let her know I have another job interview today , just in case if they were planning on hiring me and the fact the front desk lady said I would have to reapply even though the women told me she would give me a chance I just hung up she told me oh she can't transfer me to the dietary aid person but a different women all the other times has been transferring me so I could leave messages for the women and I'm glad I hung up I don't agree with why people gotta be so ignorant.

They stole cash from my grandfather after he passed away. I had to fight them for it and then it mysteriously reappeared.
I also suspect the nurses of stealing from his home (pills, valuables, and even batteries! They actually stole all the batteries. Opened the remotes and stole batteries). This could have been “local junkies” but he gave them keys and there was no sign of forced entry. Can’t prove it, but I absolutely believe it.
I also believe they forced him to sell his home so they could access his money from the unnecessary sale. The laws protect home owners, but once it’s liquidated they can take the money.
The woman in charge of all the financial matters suggested I take him on a plane and hope he has an incident when we land so that he can be placed in care in my state. This seemed reckless and inhumane.
And I actually believe they killed him with what they were giving him towards the end. No autopsy performed and they knew that would be the case.
They were uncaring and ignored patients pleas for help. He also seemed to be keeping secrets from me about his care and I suspect they were abusing him.
This all still haunts me.
Please avoid them. They are evil.
And be on the lookout for “mysterious deaths”

[review] => Giving 3 stars, rather than 2, only because there are some kind, helpful staff members and I don't want a poor experience to completely overshadow their efforts. Communication challenges are a reality here...no continuity with patient-facing personnel so requests typically require multiple attempts, and often family interference, to get answers and/or action. Dropped balls with the facility Dr. recommending a hospital procedure, but when family followed up on it 5 days later, the Nurse wasn't even aware of the order from the Dr.! They then resolved it same day, but why did it take family following up to get this going when the Dr already ordered it?! Several situations where patient/family interference was needed, often repeatedly, which does not provide for a positive experience, overall. Really hope communication protocols are reviewed to save others from such frustrating experiences.

[review] => Amazing facility; extraordinary asset to my family. I first heard about Arbor Trail (AT) when my father was in Citrus Memorial Hospital at Inverness FL for COVID. He was 87 years old at the time and came extremely close to dying, according to the doctor on duty. He survived, miraculously, and after a month in the hospital, he needed ongoing rehabilitation to build sufficient lung/breathing capacity. AT was highly recommended by a medical professional and he was subsequently admitted there. During his 3 to 4 weeks at AT, through physical therapy, daily medical attention, nutrition oversight, and excellent overall care, he left in better physical and mental health than he'd been at any point during the previous 3-years. More recently, he experienced life-threatening head trauma, suffering two subdural hematoma's, which required several days in critical care at the head trauma center in Ocala. Like before, he needed high quality rehabilitation. I didn't hesitate in arranging a return to AT. After 2-months he was released and is now at an Independent Living facility. He is functioning about as well as an 88 (almost 89) year-old man can. His latest miraculous recovery is directly attributable to the exceptional professional and personal care he received at AT. I am an objective, though often critical, evaluator who places tremendous value on pertinent customer reviews. When I take the time to write one, it is done with utmost honesty and objectivity- for the express purpose of helping the reader make informed decisions, especially concerning the health of loved ones. I have witnessed, first-hand, the exceptional rehabilitation services provided by Arbor Trail. I highly recommend their services. G. M.

We could not have been more pleased with the care given her, the compassion & encouragement shown her, and the assistance provided to us, her family members. Lots of support and activities are made available for the patients and residents. Mom's only complaint was the food -- which is understandable, when an institution has so many dietary restrictions to attend to -- though she loved the meat loaf. :o) Kudos to the entire Arbor Trail team -- you are in a challenging and often unappreciated line of work! My friend would need help setting up so she could eat or breathe easier or getting a bedpan and it took forever for a nurse to get in there to help her. She ended up having an accident in the bed and it took an hour for them to get her all cleaned up. I know times are tough right now but there is no excuse for not taking care of a patient. This is unacceptable.

[review] => This place was good for the first day and then it escalated quickly. They didn’t do anything they said they would. Wouldn’t shower her, took 20 minutes to shut off an alarm, and when we would ask why they would just get defensive. I understand they may be short staffed and I’m so grateful to anyone who tries to take care of others but this place was not it. One day my dad and I went to check on my mom and she was sick. No one told us anything or even thought of alerting us. To me, that is neglect. She had to be rushed back to the hospital. I wouldn’t recommend this place to anyone.

[review] => My Dad is at Arbor Trails for rehab. It took a couple of days for him to settle in. The staff is wonderful in assisting him. They have been patient as he settled into the daily routine. The house keeping staff keep every thing clean and tidy, the food has been good and served in a timely manner. The Therapy Staff have really been patient, understanding and consistent. The nursing staff has been great to answer questions and assist with his comfort. Last but not least the management and business staff have helped so much with helping me to under stand this process and assisting me in planning for what is next. Thanks Arbor Trails for making us feel like family and at home.

[review] => My mom has had many falls and due to osteoporosis has broken a few bones. She used to like Arbor Trails for rehab but not this most recent time. My mom has troubled keeping her O2 levels up. When I went to visit and her lips were blue I asked them to check it. It was 64! I also noted that she looked bad and was worried about pneumonia. The Dr ordered that she was to be gotten up and that was never done. We finally had her go back to emergency roomwhere she was diagnosed with pneumonia. After ER visit we took her else where for rehab where she is treated well, with respect and watched closely. We will never use this facility again.

[review] => After a terrible fall in her kitchen, jaw surgery and a stroke, my mother was at Arbor Trails for 60 days. We would give the physical therapy department a 5-Star +++ rating! They got Mom back on her feet and walking at age 93, and did everything they could to help her get back home. She made it home for Thanksgiving, her 94th birthday, and Christmas. After 100 days at home, she decided that it was time for her to become a long term resident. It is very difficult for someone that age to settle into a facility. The director, Kasey O'Leary and the admissions director, Monica Rose give her one-on-one attention and TLC every day. I often find her having coffee in one of their offices. Mom has a good long term memory but very little short term memory, so she feels safe with them. Everyone watches over Mom. The activities director, Stacy brings her to music events, and listens to audiobooks with her. Arbor Trails has 164 beds, half are rehab and half are long term residents. The facility is clean and odor-free. There is a sunny courtyard, where residents and family can visit. We planted one of Mother's favorite flowering bushes in the courtyard. The staff is very loving and professional. No one wants to live in a nursing home, but the food is good, Mother is always clean and it's the next-best thing to home. Overall, I would recommend this facility. Thanks!

[review] => From the beginning we had issues. We were told they had a lyphodema specialist for my mom. We found out they don't. They took the good compression wraps off and threw them away. They said her legs would be wrapped every other day. They weren't. We brought in her compression boots. They were put on her twice. We decided to try care and rehab at home. Within 15 minutes of getting her home we had to call for an ambulance. Her leg burst requiring 12 stiches. She also had a temp of 102 and sepsis.

Although the outside is gorgeous, and the inside doesn't look too bad either, its the care and the attention to details that are really bad. The food is TERRIBLE, and its always cold. My dad was given a shower the other night and the water was lukewarm so my dad asked her to turn it up and she said that's it, there's no more hot water. The water from the shower leaks out from the bathroom into the room and almost out into the hallway so you have to have towels ready for that. You hit your call light and you wait 30 minutes for someone to come by. No one can be bothered to talk to you. My dad calls himself an "inmate" instead of a resident. Its a good thing he wont be there that long, because if he was required to stay a long period of time, I would definitely be moving him. I hope this place gets their act together soon.
****UPDATE**** My dad has been there for 31 days now, I will say that Bonnie (director of nursing) and Monica have really gone out of their way to try and rectify some of the issues. My dad is really thriving in Physical Therapy, the rest is a work in progress. He is hanging in there!
*****UPDATE***** Dad was discharged home on Jan 15th. Today is January 28, 2019. When dad was in Arbor Trail, he got an infected spot at the bottom of his lower back incision. When his surgeon saw him on January 8th, he wanted my dad to see wound care. Arbor Trail never had my father see anyone from Wound Care. Last week I took him to wound care clinic for the area at the bottom of his surgical incision - he now has MRSA and has to wear a wound vac. There is a hole in his back the size of a golf ball (maybe bigger) where the abscess had to be removed. If they had taken my dad more seriously and gotten the area looked at sooner, I am sure it would have saved my dad a lot of pain. Just something else he has to deal with now.

[review] => My cousin was at Arbor Trails. She hated this place and cried often. As verified from friends that visited her it was filthy. The food was horrible and often cold. She was on pain medication and was terminal. She constantly had to ask for her medication. It was constantly late causing her to suffer needlessly.
There was an issue with her bowels. I was talking to her while she was on the bowl one night. An aid walked in the bathroom without knocking and asked if she was done. She said, "not yet." The aid responded by saying, "I don't have all damn night to wait for you. " Then she slammed the door. My cousin broke down and couldn't stop sobbing saying that it was always like this.
Of all the nurses I found only 2 that treated her with dignity and caring. One was a wonderful man by the name of Bob. My cousin was always happy when he was her nurse. I'm sorry that I don't remember the other nurses name.
My lovely cousin passed away there. When friends came to pick up her things 3 days later all her belongings we still in her room never packed up. To add insult to injury her jewelry was gone. My cousin had 4 young children. This was all she had left and was to go to them. Of course the police were called and all the people there swore they never saw the jewelry. Her friends certainly did. These were pieces she always had with her.
HIPA violations were also evident. When I'd call to ask how she passed the night oftentimes no one questioned as to who I was and gave me her medical information. When friends visited medical charts were in plain view on the counter, OPENED for anyone to view.
If you love your family and friends try to get them out of this facility. If they have any valuables take them with you. Someone there is stealing and others will sware to it. Stealing from the dead is one of the worst things a person could do. This is a horrible place to have to spend one's final days on Earth.

[review] => Where to I begin? My mom was there for a little over a week. Thankfully, I was able to be there for 5 of those days. This place about broke her. I was absolutely DISGUSTED, when one of the activity directors came out of a door and said hello to my mom while we were walking and she had an open beer bottle in her hand. I asked her if that was a beer bottle and she confirmed it was but that they were having a gathering in a room and that she wasn't working. I guess that makes it okay to approach a patient and their family then. This is the same woman who attempted to override my comments that was in my mom's best interest for who she is. Her snide smerks to fellow workers were seen too. Then, let's talk about my mother being served peanut and jelly when on her food paper it specifically noted she is allergic to peanuts. Thankfully she was in sound mind to know and not eat it. It was also asked of the social worker to get a paper and label the sides of the closets to help my mom and it never happened until I brought in paper and did it myself. Then, let's talk about the treatment of my mom's roommate. She is confinded to her bed with limited movement. Her granddaughter said someone was supposed to assist feeding her. The 1 and only person who did attempt to feed her brought in the food, gave her a bite, and started yelling at her claiming she was spitting at her. I came around the curtain and she could careless. I said to offer her her drink. I knew she liked that because I spent time assisting her. I was ignored. Her tray of food was removed less than 5 minutes of being in the room. Thankfully, I was able to share with her case manager who came to visit what I observed and my concern. I did observe CNAs change her that were good to her. I will say the 2 female nurses and Bob were attentive to my mom. The PT had a horrible snotty attitude. However, the OT and speech therapist were nice. In the end, I heard from various patient families their disgust too. I haven't been able to stop thinking about the treatment here and be scared for those who have to be there.

[review] => Best place to send loved ones. Staff is caring and attentive. Always smiling friendly people around to help you whenever you need. Administrator and DON always out in facility helping families and residents. Definitely a nice change from other facilities in the area. Good job Arbor Trail!

[review] => I was here for several weeks getting IV antibiotics The staff is Amazingly Supportive and extremely dependable..everyone was professional besides the night nurse ..(still don't know her name after 6weeks)...with the long bottle blonde hair..she is just too "good"to help anyone...she should of gotten a certification in nails if she wasn't going to interact with anyone..meaningfully...thank goodness for the humble hearts that DO work SO hard with our elderly population..we will all be here one day.

I couldn’t be more pleased about how the staff have taken care of me. All of my needs have been met and every concern was addressed. All of the staff here has a “can do” attitude. This is the best center I’ve ever been to. They make me feel important, heard, and I feel confident about going home. Everyone here is fantastic and I would recommend Arbor Trail to anyone who needs nursing or therapy services.
